Printing Your COVID-19 Vaccination Record/QR Code

The Golden Circle will continue to offer free printing services for the COVID-19 Vaccination Record and QR Code to assist those who do not have a printer. Come down to the Golden Circle and our Program Coordinator, Gail Wall and our Community Outreach Program Coordinator, Bonnie Lewis, will assist you.

To print off your COVID-19 QR Code you will need to bring the following

– Your Personal Health Care Number

– Your Date of birth

– The month and year of your last vaccination
